ACKNOWLEDGEMENTS, TERMS, AND CONDITIONS

1. I meet the eligibility requirements* to enroll Credit Card Balance Protection Insurance. I understand that if I provide incorrect information or fail to disclose information, my insurance may be void and treated as if never in force.
2. I understand that Credit Card Balance Protection Insurance is governed by the terms and conditions of the creditor group insurance policy and is subject to terms, conditions, exclusions and eligibility requirements. Some examples include: Pre-Existing Condition Exclusion means any claim that arises within a specified period after your insurance coverage begins may be denied coverage if it relates to a pre-existing health or medical condition. To be eligible to claim disability insurance, you understand that, at the time of claim, you must have been working 20 hours per week for two consecutive weeks. Full details can be found here.
3. I understand I have 30 days, beginning on the date this application is submitted, within which to review my optional insurance and, for any reason, cancel it and receive a full refund of any premium paid.
4. I understand that Credit Card Balance Protection insurance is optional and voluntary and can be cancelled any time.
5. I consent to Co-operators use of the email address that I have provided in this Enrolment for the purposes enrolling in Credit Card Balance Protection Insurance. We cannot guarantee complete privacy and confidentiality of any email transmissions. This includes the email text and any attachments. By authorizing communication by email, you are acknowledging that you have read and understood this notice and disclaimer and are consenting to the transmission of your personal information using email knowing the email and any attachments may be subject to unauthorized access, use or disclosure by third parties. You agree that Co-operators Life Insurance Company is not responsible or liable for any damages or losses you or any other person may suffer as a result of any breach of privacy, confidentiality or security by transmission of your personal information using email communication. If you no longer wish to communicate with Co-operators Life Insurance Company by email, please send notification to customer.service@cumis.com.
6. I consent to the Group Policyholder ("Collabria Financial Services Inc.") and my financial institution disclosing my personal information to Co-operators Life Insurance Company following the completion of my insurance enrolment for billing and other related administrative purposes . 7. The insurance for which I have enrolled is made available under one or more creditor’s group insurance policies issued by Co-operators Life to the Group Policyholder. The Group Policyholder employs or retains the services of the Group Policyholder Representative with which I am dealing with and receives 35% compensation from Co-operators Life for arranging and administering this insurance. The Group Policy Representative is not a licensed insurance agent. You may wish to contact a licensed insurance agent if you need advice about your insurance needs. Co-operators Life is a part of a group of companies owned by The Co-operators Group Limited.
8. I understand that in order for coverage to begin:
   • My Collabria Credit Card application must be processed, approved and activated;
   • I must review and confirm the above terms and conditions;
   • Click the submit button below; and
   • Receive my confirmation or coverage from Collabria.
